Abstract

The escalating burden of chronic diseases poses a critical challenge to global healthcare systems, necessitating a paradigm shift towards sustainable practices for prevention and management. This conference article explores innovative strategies and frameworks aimed at addressing chronic diseases sustainably, emphasising holistic approaches that encompass lifestyle modifications, community engagement and healthcare system resilience. Chronic diseases including cardiovascular conditions, diabetes and respiratory illnesses, account for a significant portion of global morbidity and mortality. This article investigates sustainable practices that focus on preventing the onset of these diseases through community-based interventions, public health education and the promotion of healthy lifestyles. Additionally, the article explores sustainable approaches to the management of chronic conditions, emphasising patient-centred care, integrated healthcare models and the use of technology to enhance treatment adherence. It discusses the role of telemedicine, remote monitoring and digital health tools in providing continuous support for individuals with chronic diseases, fostering self-management and reducing the economic burden on healthcare systems. Furthermore, the conference article addresses the social determinants of chronic diseases and advocates for sustainable policies that address factors such as socioeconomic disparities, access to healthy food and the built environment. It discusses how empowering communities to take an active role in their health, coupled with sustainable healthcare policies, can contribute to long-term reductions in chronic disease prevalence. The article also explores challenges related to the implementation of sustainable practices in chronic disease prevention and management, including issues of health equity, cultural sensitivity and the need for multidisciplinary collaboration. It concludes by highlighting the imperative for a global commitment to sustainable healthcare practices as a fundamental strategy in the on-going battle against chronic diseases.
